The informant on the Thomas Morrissey 1926 death record was a person named Thomas M. Morrissey. I assume that was Thomas Patrick's son, Thomas Michael Morrissey. I am guessing Thomas M. mistakenly put his mother's maiden name, Margaret Gleason, in the Maiden Name Of Mother, box 12, of the death certificate, instead of his father's mother's maiden name.
